The Examination

An SPH Experience by Hot-Initiative-2531.

I’d been hurting for three days.

Not sharp pain — more like a dull, persistent ache that started in my balls and radiated upward through my shaft. I’d felt it before, usually after going too long without release, but this was different. There was a burning when I peed. A pressure that wouldn’t go away, no matter how I adjusted.

I told myself it would pass. I drank cranberry juice. I tried to ignore it.

By day four, I couldn’t ignore it anymore.

—

The medical clinic was two blocks from my apartment — one of those neighborhood places that takes walk-ins and doesn’t ask too many questions. I’d been there once before for a flu shot, but I didn’t remember much about it.

The waiting room was mostly empty. A mother with a fussy toddler. An elderly man with a cane. Me, sitting with my legs spread wider than normal, trying to relieve the pressure in my groin without being obvious about it.

“Marcus?”

I looked up. The nurse was young, maybe in her early twenties, with light brown skin, her hair pulled back in tight braids. She had a round face and wide eyes that seemed to take in everything.

“Actually, it’s —”

“Come on back.”

She didn’t wait for me to correct her. I followed her down a narrow hallway, past examination rooms with open doors, past a supply closet and a small bathroom, until we reached the last room on the left.

“Dr. Wilson will be with you shortly. You can have a seat on the table.”

She left without closing the door.

—

The examination room was smaller than I expected. A padded table with crinkly paper, a stool, and a counter with a sink and various medical supplies. A poster on the wall showed a cross-section of the human urinary system.

I sat on the table and waited.

The ache throbbed. I shifted my weight.

Dr. Wilson came in maybe five minutes later. She was a large woman — heavyset, broad-hipped, with dark skin and a no-nonsense demeanor. Her hair was pulled back, and she wore a white coat over blue scrubs.

“So,” she said, glancing at a clipboard. “You’re having some pain?”

“Yes. In my — down there. It hurts when I pee, and there’s this pressure.”

“How long?”

“Four days.”

“Any discharge?”

“I don’t think so.”

“Sexually active?”

“Not currently.”

She nodded, made a note. “Alright. I’m going to need to take a look. Go ahead and remove your shorts and underwear, and lie back for me.”

My stomach tightened.

—

I’ve known I was small since I was fourteen.

That was the year I finally got the courage to shower after gym class. The year I saw what other boys looked like. The year I realized that the thing between my legs — barely a nub when soft, maybe two and a half inches when hard — wasn’t normal.

I’d learned to manage it—tight underwear. Strategic positioning. Avoiding situations where I might be exposed.

But this was a doctor. A medical professional. Surely she’d seen everything.

I stood up and pulled down my shorts. Then my underwear. My penis — soft, maybe an inch, barely protruding from my pubic hair — shrank even further in the cold air. My testicles, always small, had retracted tight against my body from the stress.

I lay back on the table and stared at the ceiling.

“Alright, let’s see what we’re working with here.”

I felt her hands. Warm, gloved fingers. She lifted my penis with two fingers, examining it.

“When did you last wash thoroughly?”

“I — what?”

“Under the foreskin. When did you last retract and clean?”

“I… I do. I mean, I think so.”

She made a sound — not quite a sigh, but close. “I’m going to pull back the foreskin now. This might be uncomfortable.”

I felt her fingers grip the tip of my penis. She pulled downward, exposing the head.

The cool air hit sensitive skin that rarely saw daylight. I flinched.

“Hmm.”

I couldn’t see what she was seeing. I didn’t want to.

“There’s some buildup here. Smegma. Please retract and clean daily. Warm water, mild soap. Gently.”

“Okay.”

“This is important, especially for uncircumcised men. Hygiene issues can lead to infections, which might be contributing to your symptoms.”

She continued her examination, pulling, prodding, manipulating my small penis in ways that felt clinical and humiliating at the same time. I kept my eyes on the ceiling.

—

That’s when I noticed the nurse.

She’d come back into the room — I don’t know when — and was standing near the counter, pretending to organize supplies. But her eyes weren’t on the supplies.

They were on me.

On my penis.

Dr. Wilson was still examining me, her fingers moving methodically, but the nurse’s gaze was fixed on my exposed groin. I watched her out of the corner of my eye. She wasn’t looking away. She wasn’t even pretending to be busy anymore.

Her expression was hard to read. Curiosity, maybe. Or surprise. Or something else I couldn’t name.

My penis, despite the pain, despite the humiliation, began to respond. Not fully — not even close. But a twitch, a stirring, a pathetic attempt at arousal that probably made me look even smaller.

I looked away.

—

“Alright, you can sit up.”

I did, quickly, pulling my underwear and shorts back into place. The relief of being covered again was immediate.

“I’m going to need a urine sample,” Dr. Wilson said. “We’ll test for a UTI. The symptoms you’re describing — burning, pressure — are consistent with an infection.”

She handed me a plastic container. “The bathroom is two doors down on the left. The nurse will go with you to supervise the collection.”

“Supervise?”

“Standard procedure. We need to make sure the sample is a clean catch. She’ll instruct you.”

I looked at the nurse. She was still watching me. Still had that same expression.

“Follow me,” she said.

—

The bathroom was tiny—a toilet, a sink, a small mirror. Barely enough room for one person, let alone two.

The nurse closed the door behind us.

“Go ahead,” she said. “I need to watch you collect the sample.”

My face burned. “You’re going to watch me…?”

“Clean-catch means we need to make sure there’s no contamination. I need to observe.”

I stood in front of the toilet. My hands were shaking. I pulled down my shorts again, fumbled with my underwear.

My penis had retracted almost completely. Just the tip visible, barely protruding. My balls were so tight they looked like a single small bulge.

I tried to aim. My fingers could barely grip myself — there was so little to hold.

“Take your time,” the nurse said. Her voice was soft. Almost gentle.

I couldn’t look at her. I stared at the wall, at the container, at my own inadequate equipment.

Finally, mercifully, the urine came. A weak stream, burning as it passed. I caught some in the container, my hand trembling.

“Good,” the nurse said. “That’s enough.”

She took the container from me. Her fingers brushed mine.

I pulled up my underwear, my shorts. I couldn’t get out of there fast enough.

—

Dr. Wilson was writing on a pad when I returned.

“The nurse will take the sample to pathology. Results should be back in a day or two. In the meantime, I’m going to prescribe antibiotics.”

She handed me the prescription. “Take the full course, even if symptoms improve. And remember what I said about hygiene. Daily cleaning. Retract, wash, dry thoroughly.”

“Yes, ma’am.”

She looked at me then — really looked, for the first time. Her expression was professional, but there was something else underneath. Something that might have been pity.

“Anything else I can help you with today?”

“No. Thank you.”

—

I walked home with the prescription in my pocket.

The ache was still there, but it felt different now. Sharper. More focused.

That night, I masturbated twice.

Both times, I thought about the nurse’s eyes on my penis. About Dr. Wilson’s fingers pulling, examining, exposing. About the word small echoing in my head.

Both times, I came hard.

—

I never went back to that clinic.

But sometimes, when I’m touching myself, when I’m feeling particularly pathetic, I think about that day. About the cold room. The crinkly paper. The nurse who couldn’t stop staring.

And I wonder if she thinks about it too.

The End.
